A document may use more than one of these approaches. For example: imagine a report looking into failures of procedure.
It may begin with a chronological (time) account of what has happened.
It would then perhaps go on to explain the reasons for the failures in terms of categories. These may include human error, weaknesses in systems, IT failings and so on.
It could then recommend solutions in order of importance or urgency (hierarchy).
